Table, Tent & Truck Participation and Waiver

PLEASE SIGN AND SUBMIT THIS FORM BY FRI NOV 1st 2024.

This event is FREE to vendors but if possible,

please consider making a donation so that Jam Across Georgia can continue to make an impact.  

IMPORTANT NOTES:

If you are the Contact for the Company/Organization AND WILL NOT be at the event, please ensure that your Representative(s), represent you well, by abiding with the below:

1.  This event is indoors.

2.  Two tables and chairs will be provided. Set up time is 8:30 am and you will park in the vendor parking area which will be shown when you arrive.  Food trucks will be required to park in their designated area.  If you need further assistance feel free to call Patrice 404 453-4378 or Mike 914 803-5672.

3.  Upon entering the gym, you'll be shown to your table.

4.  We ask that you do not adhere anything to the walls, so consider how to display banners etc.

5.  We are expecting about 75 - 125 guests.  If you have any raffle items, please let us know now and we greatly appreciate it. Raffles bring more awareness to your products and services and help with the event's momentum.  People love to see if they have that winning number!

6. Kindly, NO BREAKDOWN BEFORE 2:00 pm – if the JAG schedule changes, the Event host will update you.  If you need to leave early, please let us know asap.

7.  Food trucks need to provide insurance BEFORE the event.

8.  Food trucks are not allowed to dump any waste (grey water, oil, grease) in the park.  

9.  Keep your area clean and be mindful of others around you.  Professionalism is key.

I the Vendor, understand and agree as follows:

1. That I am an adult (over the age of 18) and that I am responsible for anyone accompanying me, regardless of age.

2. That I may not provide service unless this waiver is read and signed.

3. That I may be photographed during the event by Jam Across Georgia Inc. staff, volunteers and local media agencies covering the JAG Health Fair event, for which I hereby agree to have published without further release from me. This will constitute a “model release” with regard to any media bearing my likeness used for purposes of public relations or marketing of this event or any other purpose sought by Jam Across Georgia Inc. or their representatives.

4. I hereby RELEASE, WAIVE, DISCHARGE, and HOLD HARMLESS  Jam Across Georgia Inc. and their representatives (“RELEASEES”) from any and all liability, claims, demands, actions and causes of action whatsoever arising out of or related to any loss, damage, injury or death, that may be sustained by me, or to any property belonging to me while participating in the JAG Health Fair event.

5. That I specifically acknowledge that I, the Sponsor/Participant, am engaging in this service as a participant, at my own risk, and not as an employee of Jam Across Georgia Inc.
